PI Course Buy out request
- If the PI is requesting course buy out from their department this form needs to be completed in the early stages of budget development. This form is completed by PI and the course release must be negotiated and approved by the department chair & dean prior to proposal submission.

Office of Research Datasheet & Assurance Form
- Internal form for Office of Research provides data and PI’s assurances for the responsibility of conducting research.
- ISBER will prepare this form and the PI(s) will sign prior to submission.
Statement of Economic Interest (700U)
- Internal form for Office of Research. This regulation provides that research funding by certain nonprofit entities will not trigger disclosure. This form must be completed by all persons employed by UC who have principal responsibility for a research project funded or supported by a contract or grant from a nongovernmental entity. If disclosure is positive, additional forms may be required.
- ISBER will prepare this form and the PI(s) will sign prior to submission.
Disclosure of Financial Interest (800U)
- Internal form for Office of Research. Completion of this form is mandatory for all Investigators on proposals to the National Science Foundation, the Public Health Service (NIH), or other sponsors that have adopted the federal requirements for public disclosure. This information is required to comply with UC federal regulations regarding disclosure of any financial interests that would reasonably appear to be affected by the conduct of a sponsored project.

Disclosure of Financial Interest (900U)
- Internal form for Office of Research. Completion of this form is mandatory for all Investigators on proposals sponsored by PHS/NIH or other agencies adopting the 2011 PHS federal requirements. Personnel must disclose any significant financial interests related to their institutional Responsibilities at the time a proposal is submitted for PHS funding. This form may be released or transmitted to the sponsor, including federal agency representatives, and according to the California Public Records Act.

- ISBER will prepare this form and the PI(s) will complete and sign prior to submission.
Identified Personnel and Training Disclosure (900UP)
- Internal form for Office of Research. Completion of this form is mandatory for all Investigators on proposals sponsored by PHS/NIH. This form is signed by the lead Principal Investigator and identifies personnel on the project who direct or can materially influence the research, and/or are responsible for the design, conduct and reporting of such research and therefore must disclosure any significant financial interests and complete conflict of interest training.

Exception to Serve as PI/Co-PI (Form 99)
- Internal form for Office of Research. For PIs & Co-PIs not automatically eligible to serve, this form is completed and reviewed by the Office of Research to determine if an individual is eligible to serve as principle investigator. Requests for exceptions to serve as principal or co-principal investigators are issued pursuant to Research Circular B.1.

Subrecipient Commitment Form (SK-100)
- Internal form for Office of Research. Subcontract with a third-party organization performing a portion of the research project, usually to another institution. All attachments to the Subrecipient Commitment form are required at proposal stage and must be received and approved by Office of Research prior to submission.
- The subcontractor will prepare this form and an authorized official at their institution will sign prior to submission.
Subrecipient Internal Form (SK Form 800 Part A-1)
- Internal form for Office of Research. When proposing a subaward for a sponsored research project to be funded under a grant or cooperative agreement, UCSB policy requires a cost/price analysis and related information. Form is required at proposal stage and must be received prior to submission.
- ISBER will prepare this form and the PI(s) will sign prior to submission.
Multi-Campus (MCA) Commitment Form w/sister UC campus
- Internal form for Office of Research when subcontractor is another UC campus
- The collaborating UC campus will prepare this form and an authorized official at their institution will sign prior to submission.
Cost Share request, if requesting university cost-share
- Internal form for Office of Research. This procedure establishes a standard mechanism of requesting cost-sharing for extramural funding proposals. The Office of Research will receive all cost-sharing requests and will coordinate with relevant campus officials (e.g. Chancellor, Executive Vice Chancellor, Deans, Department Chairs, and Center Directors) in order to approve and construct cost-sharing packages. Should be done early in the proposal process.
